THE QUESTION

Among approved or clinical-stage compounds, which candidates reverse a pre-specified disease-relevant cellular phenotype with a reproducible dose response and an exposure-compatible margin, and which target/pathway explains the effect?
Direct exposure measurements show which anti-fibrotic candidates remain active at approved human doses.

WORK ORDER

Free-concentration correction of nominated anti-fibrotic repurposing candidates and label-dose survival

BACKGROUND

What this Apodex Lab is

Candidate molecules nominated to treat fibrosis are typically tested in cell culture at nominal concentrations between 1 µM and 10 µM. In the human bloodstream, however, oral drugs often produce unbound steady-state plasma concentrations of only 10 nM to 500 nM, and compounds with 98% to 99.5% protein binding deliver far less free molecule to tissues. When a drug requires 10 µM to reverse a cellular phenotype but reaches only 100 nM of unbound exposure at its approved dose, the margin is 0.01. Published repurposing lists match disease mechanisms to off-patent compounds across timelines of 3 to 12 years, yet roughly 75% of rare-disease repurposing filings originate from small organisations that cannot fund clinical trials without knowing whether a compound can work at its approved label dose.

Computational and transcriptomic screens nominate dozens of approved compounds by reporting a single nominal active concentration in culture medium. What remains unmeasured is how much of that nominal concentration is lost to serum protein binding in the broth, plastic adsorption on microplate walls, and membrane retention inside cells. For 28 approved or clinical-stage compounds nominated to reverse TGF-β1-driven fibroblast-to-myofibroblast transitions, no dataset pairs phenotypic reversal potency with direct free-concentration corrections measured in the same medium and plateware lot. Without rapid equilibrium dialysis and endpoint mass-balance recovery, investigators cannot separate target engagement from physical depletion, nor determine what proportion of nominated molecules remains active below label-dose unbound exposures.

The Drug repurposing for a defined disease mechanism Apodex Lab cultures primary adult human lung fibroblasts from three unrelated donors stimulated with recombinant human TGF-β1 at 2 ng/mL for 72 hours at 37 °C and 5% carbon dioxide. Using high-content fluorescence imaging of α-smooth-muscle-actin stress fibres alongside liquid chromatography with tandem mass spectrometry, the lab delivers the median nominal-to-free potency inflation factor and the fraction of the 28-compound list that retains activity at label-dose unbound exposure. If compounds with medium unbound fractions below 1% and cell-associated fractions below 5% show halved survival rates, sponsors can eliminate high-loss candidates through benchtop physicochemical assays before committing resources to phenotypic confirmation.

WHY NOW

Why the field is spending on an assumption

Transcriptomic and knowledge-graph screens nominate dozens of approved molecules per disease mechanism, reporting nominal reversal potencies typically between 1–10 µM. However, many approved oral drugs achieve unbound steady-state plasma concentrations of only 10–500 nM. Although equilibrium dialysis and mass-balance partition are routine assays, laboratories have never coupled phenotypic reversal screens with same-medium free-concentration corrections on a shared compound set using a single plateware lot. The field therefore cannot state how much of a reported micromolar potency is medium binding, plastic adsorption or membrane retention rather than free drug. Nor can researchers determine what fraction of published repurposing rankings remains active at achievable human label exposures.

WHAT WE’D BUY

What we would ask a laboratory to run

We are not buying the answer. We are buying a measurement with an interval — and the bars it will be compared against.

This work order measures 28 approved or clinical-stage compounds across primary adult human lung fibroblasts from 3 unrelated donors stimulated with TGF-β1 at 2 ng/mL for 72 hours in 384-well plates. High-content fluorescence imaging tracks α-smooth-muscle-actin stress fibres per nucleus to calculate nominal reversal potencies. Liquid chromatography with tandem mass spectrometry quantifies compound partition across medium, washed cells and plasticware, while rapid equilibrium dialysis measures unbound fraction in the same conditioned medium. The order delivers the median nominal-to-free potency inflation factor with a 95% bootstrap interval width no greater than 2.5-fold, the fraction of the 28 compounds active at or below approved-dose unbound steady-state plasma concentration with a Wilson 95% interval half-width within 16 percentage points, and regression slopes for both physicochemical predictors.

SCOPE & LIMITS

Results apply to static two-dimensional monolayers of normal adult human lung fibroblasts under TGF-β1 stimulation in low-serum medium and do not account for perfused tissue kinetics or matrix binding found in diseased fibrotic tissue.
